Transcript Release Information

Complete the attached Graduate Transcript Release Form and return it to West Babylon High School Guidance Office. Also required is a copy of your photo ID and a notarized signature.

You may also come to the Guidance Office with photo ID between the hours of 7:15am-3:00pm Monday through Friday to receive an unofficial copy of your transcript.

Please note an official copy of a transcript can only be released to a school or employer.
